Given what a complete moron Trump is, are there protections in place against him waking up one morning and 'pressing the button'? Does a Nuclear Attack require authorisation from Congress for example?

He cant just push a button as he needs joint authorisation from I think the head of defence, thats assuming it was an immediate threat and thus an immediate retaliatory strike was needed. For pre-empative strikes it will need to go through congress for approval

It's in the hands of the President and Defence Sectary as pointed out. However the President still needs to have Generals and those who operate America's nuclear weapons willing to carry the order out. They can refuse.

As far as Congress goes ? If they think a President has lost the ability to discharge the duties of President, then they can use the 25th amendment to remove the President from Office -
