Samsung Tab Pro S - 4G connection

Hi all,

I've had a search for this one but not had much luck as I'm getting conflicting info from different sites.

So I've just got me a Samsung Galaxy Tab Pro S with built in 4G connectivity. I have a mobile broadband sim from O2 (contract) installed, however I need the various settings (APN etc..) to enter into Windows 10. So can anyone advise the correct settings? The attached image shows the fields I need to complete..
